End of Abyss Stuck: Missing Levers and Wrong Map Icons
Almost every "I'm stuck" report in End of Abyss is one of seven situations, and six have a fix you can apply without leaving the room. Scan the door first: the Scanner reads "doors and what keys are needed to unlock them," so a door that wants nothing is a bug rather than a puzzle. A locked door is usually Energy Nodes or a keycard, a cleared room with a shut door is usually a bug, and damage that drains while you stand still is your suit, not your gun.

Doors that want Energy Nodes, not keys
An Energy Node Door is the one lock your keycards cannot open. The nodes are wired into the door, the Scanner draws the cable run, and every node you destroy drops the lock. The count is not constant: two on the first door after the Soldier E-19 tutorial in the Outer Sector, three at the Factory West bridge, three on each of the two Lower Levels doors guarding the Release Protocols switches. Those Lower Levels nodes sit on server racks one room back, so scan the door, follow the cables out through the doorway, and shoot what they point at.
If a door stays locked after you emptied the room, you missed a node. That is not a bug and not a skill check. Go back, scan the walls and the room behind you, count what is left. The Scanner guide lists all four critical-path doors with their node counts.
Doors that want a key you do not have
Scan the door and it names the requirement. These are the cards and keys gating progress on the launch build:

Three hide a second layer. The Operations Key is useless until the Drowned Husk stops the moving water, because the terminal past the boss makes the route north walkable. The Incinerator burns the growth over the Infirmary card. The Restricted Access Keycard is optional — chase it for the trophy, not for progress. You cleared the waves and the door stayed shut
This one is a bug report, not a puzzle. Bloody Disgusting's reviewer wrote it plainly: "I got soft locked when locked doors wouldn't open after defeating the waves of enemies, and I had several hard crashes."
The cheapest fix uses a mechanic you already have. GameSpot notes that at a restoration pod, "a save point and stand-in for Dark Souls' bonfires, enemies would reset," and GamingTrend confirms enemies respawn when you save. Resting restores the room, which restores the fight and re-arms the door. Still shut? Reload. There is no death penalty, but spent ammo and explosives do not come back, so do it at a safe room where the workbench refills you.
The lever you cannot find
GamingTrend's con list has the entry that matters most: "Easy to miss crucial objects in the dark." The same reviewer lost time to "missing a necessary lever to progress for an embarrassingly long time," and the cause is structural — most sections are "densely layered, with multiple floors overlapping and tying into each other via ladders, anchor spots, elevators, destroyable walls, and extendable bridges."
Three checks close most gaps. Scan the room you are standing in, not the one you are heading for. Check the lever is reachable at all — levers across gaps need the Grappling Hook, and anchor points only appear once you have it. And confirm your floor: a schematic covers one level, and a floor you never scanned is not on your map. Landmarks are in the sector and floor reference.
Overriding one or two Security Control Units leaves the elevator sitting there with nothing to carry you down. Power the reactors, override all three, and it moves.
You got in and now you cannot get out
If Cel stops partway along a corridor or a ladder with nothing rendered in the way, that is the invisible wall bug. IGN's reviewer hit walls that "would stop me dead in my tracks on a ladder or prevent me from doubling back," each costing a checkpoint restart of 10 to 15 minutes. Backtrack to the last Restoration Pod, reload, take the long way around. Doors with no Open prompt, and arenas that break after a boss dies, are in the invisible wall and map bug guide: a wall blocking a route you already used is a bug, a door you never scanned is by design, and a hazard you cannot survive is a mod problem.
The map says a route does not exist
GameSpot called it "an apparent bug, sometimes a door you've opened will still register as locked on the map, or a door for which you have the key won't" show correctly. The rule to live by: the icon records what the door was when you scanned it, not its live status. Walk up and try the door.
Part of this was patched — GameSpot's follow-up reported a fix roughly a day after launch, hitting launch-day players. We found no public patch-notes page and no version number for any fix, so treat the invisible walls and the cleared-room soft lock as still live. Whether a shuttle shortens the walk back is a separate question with its own seven-station list.
Elemental damage: it is your suit, not your gun
When health drains while you stand still in a lit corridor, no weapon solves it. GamingTrend is direct: elemental hazards "can only be overcome via suit mods," and "finding these suit mods typically requires the player to navigate these treacherous zones for quite a while before acquiring them." Radiation Protection sits in Factory East B2 behind the Basement Keycard, so you cross irradiated rooms to leave and fetch it, then equip it at a Restoration Pod — mods change at the pod, never at the Workbench.
Two traps sit behind that. Suit Power is your mod slot count and only Energy Cells raise it, so a mod you carry may not be equippable yet; GamingTrend notes you "will never have enough suit power to equip all your available mods." And hazards are not only radiation — the Acid Neutralizer in Waterworks B3 is a Vault pickup. The five-minute triage, in order
- Scan the door. Keycard, Energy Nodes, or nothing. Nothing means bug.
- Scan the room you are in. Levers, weak walls, cables, vents. Dark rooms are the top cause.
- Count the nodes. One survivor keeps a door shut, often one room behind you.
- Rest at the pod. Enemies reset there, which re-arms a cleared-room door for free.
- Reload last. It rewinds you to the pod, not the session; only spent ammo stays gone.

FAQ
Why can't I open this door even though I have the key? Scan it. Either it is an Energy Node Door that ignores keycards, or the map is showing a stale icon and the door opens when you walk into it. Both are reported on launch builds.
I killed everything and the exit is still shut. Bug or puzzle? Treat it as the reported soft lock: rest at a Restoration Pod to reset the room, then reload. A room that stays sealed after that is not a puzzle you failed.
Should I reload or keep exploring? Reload when a route you already walked is now impassable — invisible walls repeat. Keep exploring when you have simply not found the lever yet.
